Health Insurance Programs

The JCC is committed to helping people in our community find affordable health insurance. We help over 4,000 people get health insurance annually. You do not have to be a JCC member to take advantage of these services.

Enrollment
There is no limited enrollment period for Medicaid, Child Health Plus, or Essential Plan–you can apply any time.
When the open enrollment is closed, you can still enroll into a Qualified Health Plan if you tell us within 60 days of a life-changing event, such as:
  • Moving to New York State
  • Getting married or divorced
  • Having or adopting a child
  • Becoming a citizen, national or lawfully present individual
  • Losing employer insurance
  • Losing eligibility to be on a parent's health plan
Community Health Insurance Programs (CHIPS)
The CHIPS Staff are trained Navigators, certified through the New York State Department of Health. They assist individuals and families in enrolling into public health insurance programs (Medicaid, Child Health Plus and Essential Plan) as well as in Qualified Health Plans through the New York State of Health Marketplace.
The multi-lingual Navigator staff can assist people under the age of 65 with eligibility, enrollment, plan selection and health literacy. We speak English, Spanish, Russian, Ukrainian, Polish, Yiddish, Korean, and Romanian.
Services are available at various locations with weekend hours offered. There is no cost to enroll, although premiums and co-pays may apply, depending upon program.                                                      
All services are free and confidential!
CHIPS Navigators located in your community are available to assist you in applying for health insurance.

​Child Health Plus (CHPlus) 
New York State sponsored free/low cost health insurance program for children. Depending on your family’s income, your child may be eligible for Child Health Plus. The Program is available through a variety of managed care plans and is accepted by dozens of providers. To be eligible children must be under the age 19 and be residents of New York State.  Children are eligible regardless of immigration status.

  • Prescription drugs
  • Physical exams 
  • In/out patient hospital
  • Immunizations
  • Dental and vision care
  • X-ray and lab tests
  • Speech and hearing
  • Outpatient surgery
  • Substance abuse treatment
  • Emergency care
  • Mental health treatment
  • Chemotherapy/hemodialysis
Medicaid (MAP)
New York State sponsored free health insurance program that offers low income adults and children complete coverage for all their health care needs. Eligibility is determined by income, household size and immigration status. Medicaid eligible clients are counseled on their choice of managed care plans and how to access their benefits. Most Medicaid eligible clients are able to continue seeing their current primary care physicians. You choose your health plan.
Essential Plan (EP)
New York State sponsored free/low cost health insurance program for individuals between the ages of 19 and 64 who don’t qualify for Medicaid. EP’s provide same essential health benefits, with no annual deductible.
*Whether you are enrolled in Child Health Plus, Medicaid or Essential Plan you will have to renew every 12 months, which our office can process for you.*
Available Plans Through Medicaid, Child Health Plus and Essential Plan:
Affinity; Emblem Health; HealthFirst; Empire; Fidelis; MetroPlus; UnitedHealthcare Community Plan
Qualified Health Plans (QHP)
Affordable health plans offered through the marketplace for individuals between the ages of 19 and 64 who are not Medicaid or Essential Plan eligible. QHP’s provide ten essential health benefits, follow established limits on cost-sharing (like deductibles, co-payments and out-of–pocket maximum amounts) and meets other requirements.

  • Prescription drugs
  • Hospitalization
  • Preventive and wellness services and chronic disease management
  • Maternity and newborn care     
  • Mental health and substance abuse disorders    
  • Rehabilitation and habilitation services/devices
Patient Activation Project
The Patient Activation Project, in partnership with Staten Island Performing Provider System (PPS), is an island-wide initiative to improve health and health literacy for the Staten Island community. 
​JCC participates in this program in order to:
  • Connect our community members to health insurance options
  • Link individuals who are on Medicaid or uninsured to high quality, culturally sensitive care on Staten Island
  • Minimize avoidable chronic illnesses
  • Reduce avoidable emergency room visits
  • Build healthy neighborhoods
  • Reduce preventable hospital admissions and readmission
